North Phoenix · care questions

What happens at the first assessment?

Bring the problem you want help with. Have a list of the medicines you take now, along with your questions about the proposed treatment. Keep the conversation tied to your health, not a package name.

Put the useful details together

A short note can keep the visit focused. Use your own words rather than trying to sound medical.

  • Write what you want to change and how it affects your day.
  • List medicines and supplements, plus known allergies.
  • Bring existing records or test results if you have them.
  • During a medicine discussion, say whether you are pregnant, breastfeeding or planning a pregnancy.

A form can start the conversation

Licensing boards make up the Federation of State Medical Boards. It says a fixed online form alone does not give enough information for care. A form may help if later questions depend on your earlier answers. The treating person must still be able to gather the details needed for your assessment by asking further questions.

  • This document gives advice to licensing boards. Each state has its own prescribing rules.
  • In the October 3, 2026 Arizona rules, an MD—a medical doctor—needs an appropriate examination or an established care relationship before prescribing. The examination may happen through telehealth. Arizona law lists exceptions. When a doctor relies on an exception, have them name it and explain why your visit fits it.
  • Ask how to discuss details that do not fit the form.

Ask what any test is for

The boards’ online-care advice says the treating person must arrange any needed tests or refer you for a visit in person. It does not say everyone needs the same tests.

  • What question will this test help answer?
  • Who orders it, reads it and explains the result?
  • What happens if treatment is not a good fit?
  • Can I have the plan and costs in writing before deciding?
